These match caregivers to shifts. Everything starts as Yes (AxisCare assumes Yes when unset) — just flip the No's. The staffing coordinator enters these into AxisCare from the Staffing Hub checklist.

This form does NOT create the official job offer — the applicant only receives their offer and onboarding paperwork after you also add them to Viventium (you'll get a copy-paste block on the next screen). The welcome text tells them Viventium is coming, so skipping that step leaves them waiting on a message that never arrives.
